Peak Form Medical Clinic (an occupational medicine and physical therapy practice) has expanded to its new Boulder location. We’re conveniently located in South Boulder, right next to Lucky's Market.
To celebrate we are having a Grand Opening and Open House Party!
Come enjoy catering by Sushi Zanmai and win fabulous door prizes! The Peak Form staff will be there to greet you and give you a tour of our progressive new occupational health facility.
The first Peak Form Clinic opened in Boulder in 2003 and we’re proud to still be locally owned and operated.
